Completed Nomination Forms must be submitted one of the following ways,  and no later than 1 week prior to the scheduled meeting. 

Forms may be filled out and submitted online (below), OR

download the PDF version and scan and email to:

info@100womenwhocarecaledon.com.

If you have made a commitment as part of a team, each

team may only submit one nomination per meeting and

only one member per team can present to the group

 if this charity is selected.
